Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

Beechwood, Funeral, Cemetery and Cremation Services, (Beechwood) acknowledges and understands the risks related to Covid-19.

To meet the needs of our families, clients and guests, Beechwood has taken the necessary precautions recommended by Ottawa Public Health and the Bereavement Authority of Ontario to provide a safe and clean environment for our guests and staff. We are committed to keep a clean and sanitary environment for all our families, clients and guests. We have undertaken enhanced cleaning protocols and procedures to increase the safety our clients and staff.

We ask that all visitors adhere to the new protocols and procedures, including:

  • Pre-registering attendance for all funeral services, for public health reason
  • Submitting to a mandatory temperature check before entering the Beechwood National Memorial Centre (building)
  • Wearing a mask within the building
  • Sanitizing hands upon entry into the building
  • Adhering to social distancing and limited physical contact outside your household members
  • Adhering to the directional signs
  • Adhering to any and all directions from Beechwood staff

Beechwood, to the best of our abilities, will meet all the requirements for post covid-19 services mandated under the federal, provincial and municipal public health guidelines.

By signing or registering digitally to this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the services provided by Beechwood and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I understand that the risk of becoming exposed to or infected by COVID-19 at Beechwood may result from the actions, omissions, or negligence of others, and myself; including, but not limited to, Beechwood employees, suppliers, guests and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance of a service at Beechwood.

On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Beechwood, its employees, representatives and suppliers, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Beechwood, its employees, representatives and suppliers, whether a COVID-19 infection occurs before, during, or after participation in any Beechwood service.

Please note:

  1. Each guest must register individually. Please do not register as a group.
  2. Each guest must register individually for the visitation, the funeral service, the interment and the reception (when and if applicable).
